First Visit

Your First Visit

We thank you for choosing Advanced Dentistry of Coral Springs for your dental needs. During your first consultation, we will perform an extensive examination to make sure we catch any problems areas early, if there were to be any.  Dr. Buitrago will then go over with you your exam results and recommend treatment options.  In most instances, work can be performed on the same day of your visit.  For more complex treatments, we would need to schedule for a second visit to finalize any urgent work that needs to be performed. 

We know your time is important…

In order to make your visit run smoother, please have the following documents ready at the time of your initial visit:

• Any referral slip and or x-rays (if applicable)
• List of medications you are presently taking
• New patient form/s filled out (can be downloaded below)
• Dental insurance card

NOTICE:

Any patients under the age of 18 needs to be accompanied by parent or guardian at time of consultation.

PRE-SURGERY NOTICE:

Please let our office know if you have a medical condition that may be of concern prior to oral surgery (example: diabetes, high blood pressure, artificial heart valves, rheumatic fever, etc.) or if you are currently taking any blood thinners such as: aspirin, heart medications, anticoagulant prescriptions.

Thank you for your interest in our practice.  In order to facilitate your visit with us, we have made our new patients forms available for you.  Before your initial visit, please take some time to fill out all forms as this will speed up your office visit.
